               Operational Works Application – new driveway to property

Please find attached the following documents in support of an Operational Works
application for a new driveway at the above address.

   -   DAForm 1 – v1.3
   -   Tod Drawings ref 20651-C1000 & C1100
   -   Email from Unitywater confirming acceptance of reduced cover over sewer main
       provided driveway support piers and installed to transfer load below sewer (as per
       the attached drawings)

Please note that due to the levels of the existing verge the cover over the existing sewer is
reduced below normal Unitywater requirements and there is no way of lowering the sewer
due to downstream levels. Unitywater have review our drawings and have no objections to
the reduced cover provided the vehicle loading is not directed on to the sewer main. This
has been achieved by the addition of two blade piers either side of the sewer for the full
width of the driveway. Due to the above constraint the driveway width in the road
reserve/verge has been limited to 3.5m.

Please note that a large tree on the front boundary will need to be removed to facilitate the
driveway construction. This has been noted on our plans.

Could you please advise the fee applicable for this application so we can arrange
payment.
