Please find enclosed an Other Change application for a Material Change of Use to be assessed by Noosa
Council. In support of this application please find enclosed:

   ●   Landowner’s consent;
   ●   DA Form 1;
   ●   Proposal Plans prepared by Blackburne Jackson Design.

Payment of the relevant fee being $6,274.00 will be made once a reference number and invoice has been
