       ATTENTION: The Chief Executive Officer

Dear Sir

GENERALLY IN ACCORDANCE REQUEST
DEVELOPMENT PERMIT FOR MATERIAL CHANGE OF USE - HEALTH CARE SERVICE,
HOSPITAL AND HOSPITAL (INFUSION CENTRE) AND RECONFIGURATION OF A LOT

36 - 40 HOFMANN DRIVE, NOOSAVILLE

Further to Councils approval of 23 October 2025, more detailed design for the building and
uses has necessitated some adjustments to plans. These adjustments comprise:

1. Addition of a medical gas room for the Day Hospital to the ground floor carpark.

A medical gas plant room has been added to the ground floor, within the undercroft carpark
with access to the loading bay. The gas room has been located within a space approved
as a carpark, with the carpark relocated adjacent to a smaller plant room under the ramp
to the upper level. Refer plan extract below

The gas plant room does not constitute GFA, will not be visible from the street and
   maintains approved car parking numbers.

2. Relocation of the Building 2 plant deck.

The building 2 plant deck has been relocated from a freestanding structure above carparks
to the building roof. Refer plan extracts below.

             Approved Proposed

The relocated plant deck is located at the rear of the site, will be screened, and will remain
below the maximum building height. While the plant deck is noticeable on the front elevation
view, due to the location at the rear of the site and building height, the plant deck will not be
visible from the street

In this regard the updated plans are submitted for Councils generally in accordance review.

A summary of the extent of the plan changes is as follows:
