RE:       Development Application for Material Change of Use - Multiple Dwelling (3 Units)
          31 Tewantin Road, Cooroy
          Lot 106 on C5604

On behalf of the applicant, Fastwin Investments Pty Ltd, and in accordance with section 50 of the Planning Act
2016, please find enclosed a ‘properly made’ development application for a Material Change of Use to establish
a Multiple Dwelling on the abovementioned site.

Please find enclosed the following in support of the application:
      •   Completed DA Form 1;
      •   Signed Landowner's Consent Form; and
      •   One (1) copy of ADAMS + SPARKES Town Planning Report, including supporting information.

Payment of $7,713.00 being the applicable fee for three (3) Multiple Dwelling units under the 2022/2023
Schedule of Fees and Charges will be paid upon receiving an invoice from Council.
