situated at 40 Cullinane Street, Tewantin QLD 4565, we hereby change the application via an addendum to include approval for a
Development Permit for Operational Work (Electrical Reticulation).

In support of the addendum, please find attached:

     •      Design for Electricity Supply Plan prepared by Millwell Services Pty Ltd.
