Dear Mark
RE: PROPOSED DRIVEWAY TO 22 HOOP PINE ROAD, EERWAH VALE
OPERATIONAL WORKS APPLICATION
We refer to our recent discussions regarding the provision of a driveway/property
access at the above address and enclose the following documents in support of
a Development Application for Operational Works.
• IDAS Form 1;
• IDAS Form 6;
• IDAS Checklist 4;
• 2 copies of Tod Consulting Drawings 15633 C01 to CO?;
• Our Client's cheque for the Operational Works Application fees for
$1,614.00.
Please note that the driveway is located within the Hoop Pine Road reserve but it
is intended to be a private access only and not public road. Notwithstanding, we
have provided a turning area at the bottom of the drive as requested in case a
member of the public does drive down by mistake.
The subject property is located in the Sunshine Coast Council region, however
the majority of the work is with the Noosa Shire Council region, hence our
Application to you. Sunshine Coast Council have been consulted and have
advised that they do not require an Operational Works Application as the work
within their boundary can be considered self assessable. They have advised that
a permit to undertake work within the road reserve will be required only.
Note that we are unsure whether this Application needs to be referred to SARA
for clearing of native vegetation. We have ticked Item 3 in Section 2.1 of IDAS
Checklist 4 as we believe the clearing is mentioned in part 5(g) of Schedule 24,
Part 2 of the Sustainable Planning Regulation 2009. As such, this aspect of the
Operational Work is not assessable development, however your confirmation of
our assessment would be appreciated.

Notes for completing this form
• Section 261 of the Sustainable Planning Act 2009 prescribes when an application is a properly-made application.

assessment. Where schedule 3 identifies assessable development as "various aspects of development" the
applicant must identify each aspect of the development on Tables A, Band C respectively and as required.
Question 6
• Section 263 of the Sustainable Planning Act 2009 sets out when the consent of the owner of the land is required for
an application. Section 260(l)(e) ot the Sustainable Planning Act 2009 provides that if the owner's consent is

Question 11
• The Building and Construction Industry (Portable Long SeNice Leave) Act 1991 prescribes when the portable long
service leave levy is payable.
• The portable long service leave levy amount and other prescribed percentages and rates for calculating the levy
are prescribed in the Building and Construction Industry (Portable Long Service Leave) Regulation 2002.
IDAS fonn 1-App/icalion delails
