In the event of any emergency on site where Emergency Services must attend, it is the Emergency Officer or Supervisor’s responsibility to ensure that Emergency Services are directed to the location of the emergency in the shortest possible time. To facilitate this:

Clear direction must be given to Emergency Services as to the location of the site – Lake MacDonald Drive, Cooroy
A nominated person must be sent to the access gate or other determined meeting point to escort Emergency Services
Route from the access gate to the location of the emergency must be made clear for Emergency Services
Emergency Services are to be escorted from the access gate to the location of the emergency

SUSPECTED CASE OF COVID-19

In the first instance, workers should not attend site if they have COVID-19 symptoms and should seek medical advice
If a worker presents to site and has COVID-19 symptoms, they are to immediately be removed from site and seek medical advice
No worker is to return to site after presenting with these symptoms until a medical clearance is given and documentation forwarded to the Supervisor
If a worker is confirmed positive for the COVID-19 virus the ‘Shadforth COVID-19 Confirmed Case Management Procedure for Projects’ is to be followed

PERSONAL INJURIES
Assess the site and make it safe, before rendering assistance
Attend to the injured person, by first attempting to gain a response.
If no response is forthcoming commence required first aid.
If there is a possibility of neck or back injuries the person should not be moved, unless there is danger to their life - Call an Ambulance if required. It is always advisable to call for an Ambulance rather than transport the injured person to help.
First aid must be continued until an ambulance or some other form of help arrives.
If the person is conscious or gains consciousness during first aid, make them comfortable and keep them under observation continually until help arrives.
Complete an Incident Notification Form and obtain statements (if required), in accordance with the Incident Management Procedure IMS-02-09.
FIRE
Immediately alert other workers by verbal warning and/or use of air horn
If the fire is considered controllable, attempt to extinguish the fire. A decision should be made within the first minute whether to continue with this approach.
If the fire is considered uncontrollable, contact 000 immediately and move all personnel to the evacuation area. Plant should be moved, if safe to do so.
Ensure all personnel are accounted for at the evacuation area.
Complete an Incident Notification Form and obtain statements (if required), in accordance with the Incident Management Procedure IMS-02-09.
TRENCH COLLAPSE
Clear all unwanted workers away from the area, as per site evacuation plan.
Evaluate the situation, contact Site foreman and relevant safety personnel
Contact Emergency Services (000), if required
Try to locate the victim looking for signs of survival, with equipment such as tools or a shovel.
Batter the sides of the trench in the collapsed area or insert shoring where possible, to protect the victim and rescuers.
Remove collapsed soil with shovels or if it is a very deep trench, with a machine. If machine is used, take extreme care to avoid further injuries.
Allow no-one on top of the collapsed soil, as this will increase the pressure on the victim.
If the trench is over 1.5m deep, rescuers are to wear safety harnesses and lifelines which must extend to the surface.
When rescuers are near the victim, hands should be used to remove the soil if possible. If shovels have to be used, these are to be used with caution.
When the victim is located, clear soil from head and chest, and check for breathing and a pulse. Check for injuries and apply first aid where necessary.
If no signs of life are present, start Cardio-Pulmonary Resuscitation (CPR).
If the victim’s chin is pressed onto his/her chest, tilt the head back as soon as possible so as to open up the airway to the lungs. This can be done while other rescuers are freeing the soil from the upper part of his/her body and before starting CPR.
Do not remove the victim by tying a rope around them and pulling the rope.
Where possible, leave the victim in the trench until the ambulance or qualified medical staff arrives.
PLANT ROLLOVER
Clear all unwanted workers and plant away from the area
Evaluate the situation, contact Site foreman and relevant safety personnel
If required Contact Emergency Services by telephone (000).
If there is an injury to a worker follow personal injury procedure above.
If the emergency involves spills, follow hazardous chemicals or hazardous substance spills procedure.
HAZARDOUS CHEMICAL / SUBSTANCE SPILLS
Clear all unwanted workers and plant away from the area as per site evacuation plan.
Immediately contact Emergency Services by telephone (000) and the site foreman and relevant safety personnel
Isolate spills where possible
Contain spill with site spills kit
ELECTRIC SHOCK / ELECTROCUTION
Isolate supply. Avoid contact with the victim. Remain a safe distance until supply is isolated
Avoid becoming another victim due to the risk of electric shock, extreme heat, toxic fumes or smoke.
Send for help as soon as the situation allows
Assess the situation and rescue the victim as quickly as possible.
Move the victim to a clear, safe area to allow for assessment/treatment.
Perform resuscitation/treatment of injuries - burns, bleeding, etc.
Place the victim in the recovery position, await further medical help.
Rescuer to remain with the victim and continually monitor.
DEEP EXCAVATION / CONFINED SPACE
Emergency Rescue Procedure IMS-02-36.
Secure area and notify Emergency Services and Site Management.
Where a gas alarm is sounding, do not enter until readings have been confirmed safe for entry.
Injured worker/s should only be moved or extracted if in immediate danger. (re-assess if conditions change)
Injured worker to be assessed by first aid officer or emergency services.
If the injured worker is not in immediate danger, first aid treatment should be provided where possible.
Where possible use the Davit Arm to extract injured workers.
Worker entering space as rescuer to wear harness and rescue line.
Where Davit Arm is not appropriate, harness and excavator are to be used.
For excavator rescue follow;
Notify the excavator operator immediately to position the excavator to attach the lifting shackle.
While the excavator is being prepared, the crew leader checks that all safety rigging is done, and all the required safety equipment is available.
With two rescuers in control of the lift, direct the lifting lanyard to a position that is above and as close as possible to the injured worker. A designated worker on the ground guides the operator. The designated worker must make sure that when the lanyard reaches the right elevation the operator is signalled.
Rescuer #1 gets into a position to reach the injured worker.
Rescuer #2 lowers the line that will be used to retrieve the worker. Rescuer #2 attaches an extra lanyard to the line if required.
Rescuer #1 assesses the injured worker for injuries and then decides how to proceed (i.e. treat injuries first, guide the injured worker into the harness, or fits the harness to the injured worker).
Injured worker to be lifted from the trench / confined space to a safe location.
Once the injured worker has been brought to a safe location, administer first aid. Treat the person for suspension trauma and any other injury.
Arrange transportation to hospital. A designated worker must accompany the injured worker to hospital.
Emergency and PPE equipment required for rescue & works
Gas detector on worker in excavation or confined space
Harness on all workers, while entering and exiting
Worker to be attached to Davit Arm while entering & exiting
One harness with rescue line and spreader bar to be available for rescue
If a harness is the method used, keep the following points in mind.
Perform a harness rescue only when it is not possible to use conventional equipment (like a ladder or ramped entry) to safely rescue the worker.
Never attach multiple workers to one lanyard.
Ensure that a competent worker inspects the rescue plant/equipment, prior to lifting rescuers.
Always equip the excavator with a fail-safe mechanism (by engaging crane mode) to prevent the boom from descending in the event of a power/system failure.
Maintain an adequate means of communication between the rescuers and the excavator operator at all times.
Make sure that all rigging used to attach the harness to the hook has a safety factor of 10 against failure.
Do not allow excavators to travel while worker attached.
Do not suspend workers during high winds, electrical storms, or other adverse conditions that could affect the safety of personnel on the platform or in the harness.
