assessable application for a development permit for a material change of use for an
extension to an aged care facility on land located at 186 Cooroy Noosa Road, Tewantin
and formally described as Lot 2 on RP177554.

The applicant for this application is NoosaCare Inc and all correspondence should be
addressed to the applicant c/- Buckley Vann Town Planning Consultants.

In suppott of the application, please find attached the following documentation:
