OPERATIONAL WORKS 13/0022 411 Black Pinch Road — Cootharaba
On Maintenance: Roads, Drainage

The above works are now complete and we request that the above works be accepted on
maintenance as of 21* May 2014.

As noted on site the Telstra cable has been installed to the turn area adjacent to house site.
Please arrange for the performance bond of $10,000 to be returned to the owner.

We certify that we have supervised the roadwork’s and drainage works which have been
completed in accordance with good engineering practice, the approved plans, OPW13/0022

permit, conditions, specifications, and to Council requirements.
