             Dear Sir/Madam,
             We submit for your approval the Operational Works application for the above-mentioned
             project. The proposed works involve the civil engineering works in accordance with relevant
             conditions of the higher order related approval (Council Ref: MCU22/0202).
             As part of the application, the following documents are provided:

                  •    DA Form 1
                  •    Operational Works civil drawings prepared by Bax Engineering (plan set 458-001
                       revision A)
                  •    DA Form 2 (Parts 4 to 6 completed in accordance with DA Form 1 Part 8)
                  •    Katie Rose Hospice Letter of Request

             No application fees have been calculated for the works, and it is humbly requested that the
             council consider discounting such fees in accordance with the letter provided by Katie Rose
             Hospice and attached to this application.

             It is confirmed that the engineering drawings have been designed in accordance with the
             Noosa Council Planning Scheme’s Infrastructure and Operational Works Code.

             We request notification of a council reference number and the issue of a tax invoice upon
             receipt of the application package and that any correspondence be forwarded to me at
