Dear Council,

Please find attached in support of an Operational Works application for new “non-standard” crossover and
driveway at the above address the following information:

    1. DA Form 1; and
    2. Site plans of the proposed new vehicle crossover

The application is required due to condition 9 of RAP22/004 due to driveway exceeded width and it will
impact existing on-street parking/road furniture.
