for a Material Change of Use to establish a Multiple Dwelling (4 Units), located at 17 Myall Street,
Cooroy (Lot 22 on C5605)


In support of this application please find the attached:
    -    Completed DA Form 1;
    -    Signed Land Owners Consent Form; and,
    -    Development Assessment Report prepared by URBN Town Planning (including
         Attachments 1 - 6).


Council’s Application Fee of $8,751.00 will be paid upon receipt of Council’s invoice.
