I, Graeme Arbuckle being the registered owner of the property described as Lot 2 on RP
7126723 and Lot 1 on RP 28724 (subject to survey) hereby grant NBN Co Limited the right

to lodge on my behalf any applications required for Town Planning and/or Building Approval

for the establishment of telecommunications infrastructure on the abovementioned property.

Please note under the Sustainable Planning Act 2009 the guidelines for owners consent are;


       .     If there are multiple owners a signature is required from each one
       .     The ACN Number and if the owner of the land is a company two (2)directors of the

             company or a director and a company secretary
       .     OR if a sole director and no Secretary, the owner consent must clarify that the person is
             the sole Director is also the sole Secretary.
       .     A company seal can be used if witnessed by two company directors or a director and a

             company secretary.
       .     If the land is leased the lessor must sign the form,
       . Where the owner is identified as a Body Corporate, a Body Corporate Seal and Body
         Corporate resolution consenting the proposal & two signatures (one of which must be the
         chairperson OR
       .
         Body Corporate Seal and a letter of consent signed by chairperson and secretary OR
       . A signed letter of consent from each lot owner covered by the Body Corporate.
