On behalf of the applicant, David Ezrine, and in accordance with section 50 of the Planning Act 2016, please find
enclosed a properly made development application for a Reconfigure a Lot for a Boundary Realignment (2 Lots
into 2 Lots) on the abovementioned site.

Please find enclosed the following in support of the application:
      •   Completed DA Form 1;
      •   Signed Landowner's Consent Form; and
      •   One (1) copy of ADAMS + SPARKES Town Planning Report, including supporting information.

Payment of $3,637.00 being the applicable fee under the 2025/26 Schedule of Fees and Charges will be paid
upon receiving an invoice from Council.
