84.34 Chief Power, as an employer to give the insurer written notice in the Section 133A Workers’
Executive approved form if:- Compensation and
Officer (a) a worker asks the employer for compensation for an Rehabilitation Act
injury sustained by the worker; or 2003
(b) the employer pays the worker an amount, either in
compensation or instead of compensation, that is
payable by the employer as a self-insurer or
WorkCover under the Act for an injury sustained by the
worker.
