Please find enclosed a combined Development Application for Multiple Dwelling (19 units), Food and
Drink Outlet, Office, Indoor Sport & Recreation and Health Care Services and associated Operational
Works.

In support of the application, please find attached:

    •     DA Form 1;
    •     Development application report prepared by Project Urban;
    •     Proposal Plans prepared by Archiology Architects;
    •     Stormwater management plan, Music model, Engineering plans and Engineering report
          prepared by Alta Engineers;
    •     Arboriculture Report prepared by Tree Solutions; and
    •     Landscape Concept Plans prepared by Project Urban.

Please note that we calculate the fee for the code assessable material change of use application will be
$29,591.00 based on 19 multiple dwelling units and the highest fee payable for the range of commercial
uses proposed in the application. See Table below.
