RE:    Request for Minor Change to Development Approval (23185 DA) relating to a Development
       Permit for Material Change of Use - Extensions to Existing Shop at 12 Maple Street Cooroy,
       described as Lot 806 on C5602

In accordance with section 78 of the Planning Act 2016, please find enclosed a request for a Minor Change
to the Development Approval (23185 DA) at the abovementioned property.

This request is made on behalf of Traders Lane Pty Ltd, being the applicant for the original application.

In support of the application, please find enclosed:
     + Planning Act Form 5; and
     + Proposal Plans (Attachment 1);
     + Owners Consent Form (Attachment 2);
     + Flood Search Certificate (Attachment 3).

Payment of the minor change application fee will be made upon Council issuing the relevant Tax Invoice.

Background

Site Description

The subject site is located at 12 Maple Street, Cooroy, and is described as Lot 806 on C5602. The site has
an area of 1,011m² and has frontage to Maple Street to the east and to Council-owned car parking areas to
the north and west. The site is improved by an existing commercial building accommodating a range of
tenancies.
