                                          Operational Works Application – Tree Removal
                                     In relation to Extension to Existing RSL Club MCU16/0079
                                                       5 Memorial Ave, Tewantin

Dear Sir / Madam,


Please find enclosed an application for operational works being the removal of one (1) tree internal to the RSL club site. The removal
of the tree is required for the establishment of a replacement sewer line (150 PVC-U type B4) and associated infrastructure as
shown on the attached decision notice issued by Unity Water, 18th November 2016.

As can be seen on the Unity Water approved plan (‘Sewer Reticulation Layout Plan’ by vT Consulting Engineers with reference
16144/C003 rev.D dated 15/1/2016) the single tree requiring removal is internal to the site and is in proximity to the sewer location.
As such, to avoid any further damage to the sewer and subsequent costs, this tree is required to be removed. The anticipated cost
of removal is E$2000.00.
