Re:        Proposed Material Change of Use & Operational Works for Two (2) Type 2 Duplex’s
           111 & 113 Gympie Terrace, Noosaville
           Lot 6 on RP79817 & Lot 18 on RP66590

In accordance with section 260 of the Sustainable Planning Act 2009 (SPA), please find enclosed
a ‘properly made’ development application for Proposed Material Change of Use for Two (2) Type 2
Duplex’s, on the abovementioned site.

Please find enclosed the following in support of the application:
     Completed IDAS Forms Part 1, Part 5 and Part 6;
     Signed Land Owner's Consent Form;
     One (1) copy of ADAMS + SPARKES Town Planning Report; and
     Payment of $3,703.00 for each Duplex, being the applicable fee under the 2016/2017
          Schedule of Fees and Charges, will be paid by the applicant’s directly.
