Operational Works Application - Stage 3

On behalf of the applicant, Palm Lake Works Pty Ltd, we submit for Council’s approval an Operational Works Application
for 19 Trading Post Road, Cooroy.

We enclose the following to support this application:
e IDAS Development Application Forms 1 and 6; and
e Engineering drawings prepared by Calibre Consulting (Qld) Pty Ltd.

In addition to the above information, we advise that the design intent has been based on the previous specialist technical
reports:

e Stormwater Management Plan — Calibre Consulting (16-001014-01A September 2016 and December 2016
modifications)

e Stormwater Management Construction and Maintenance Plan (16-001014-02A September 2016)
We also advise:

e The Project Superintendent will be Brad Thompson, Calibre Consulting (Qid) Pty Ltd with the Superintendent's
Representative to be Anthony Wright.

e Testing will be in accordance with Calibre Consulting (Qld) Pty Ltd Specifications and Council’s Planning Scheme
Policy;

e The Q Leave form has been forwarded to the applicant for payment. A receipt will be forwarded to your office in due
course; and

¢ Operational Works design is based on Noosa Planning Scheme Policy.

Please confirm receipt of submission and issue an invoice for payment of application fees to Palm Lake Works Pty Ltd
