In accordance with Section 50 of the Planning Act 2016 and on behalf of Andema Pty Ltd, please find
attached a ‘properly made’ development application for a Material Change of Use for Business Uses
(Commercial Business Type 1 - Office, Entertainment and Dining Business Type 1 - Food and
Beverage and Retail Business Type 2 - Shop and Salon) at 6 Heron Street, Peregian Beach, formally
described as Lot 69 on P9313. We enclose the following information in support of the application:

•   DA Form 1 (Development Application Details);

•   Planning Assessment Report addressing the applicable assessment benchmarks, including
    attached specialists’ reports:
       o   Appendix A – Title Search;
       o   Appendix B – Planning Scheme Code Assessment;
       o   Appendix C – State Assessment and Referral Agency Assessment;
       o   Appendix D – Architectural Plans prepared by Cox Architecture;
       o   Appendix E – Architectural Design Rationale prepared by Cox Architecture;
       o   Appendix F – Acoustic Assessment prepared by Renzo Tonin and Associates; and

•   Land owner’s consent.
