    ➢   Completed DA Form 1 and Signed Landowner’s Consent Form; and
    ➢   A digital copy of Planning Report – 100238 prepared by Murray & Associates (QLD) Pty Ltd,
        inclusive of Appendices A-F.

Please note that Council’s Assessment Fee, being $4,654.00 will be paid directly to Council by the
applicant following submission.
