Ltd, please find attached a ‘properly made’ development application for a Development Permit for a
Material Change of Use for Hospital and Health Care Service 47 Goodchap Street, Tewantin. We
enclose the following information in support of the application:

•   DA Form 1 (Development Application Details).

•   Planning Assessment Report addressing the applicable assessment benchmarks, including
    attached specialists’ reports.

•   Relevant plans of the development.

•   Landowner’s consent; and
The relevant application fee has been calculated based on the 2022-2023 financial year fees and
charges schedule will be paid online upon lodgement of the application, once an application reference
number is confirmed.
