In relation to the abovementioned property, on behalf of Coffee Rock Investments P/L, we hereby submit an
application for an Operational Works Permit in accordance with s.51 of the Planning Act 2016 and the higher
order approval MCU21/0164 (Condition 11).

Please find enclosed the following documentation to facilitate Council’s assessment of the submission:

         • DA Form 1,
         • Certificate of Title, Company Extract and signed Landowner’s Consent,
         • Signed Body Corporate’s Consent, and
         • Appendix A Proposed Landscape Plan prepared by Frank Macchia WD8.00, revision A, dated
            11/10/2022.
