hereby grant NBN Co Limited the right to lodge on our behalf any applications required for Town Planning
and/or Building Approval for the establishment of telecommunications infrastructure on the

abovementioned property.

Please note under the Sustainable Planning Act 2009 the guidelines for owners consent are;

ef there are multiple owners a signature is required from each one

e The ACN Number and if the owner of the land is a company two (2) directors of the company
or a director and a company secretary

e ORifa sole director and no Secretary, the owner consent must clarify that the person is the
sole Director is also the sole Secretary.

e Acompany seal can be used if witnessed by two company directors or a director and a
company secretary.

» If the land is leased the lessor must sign the form.

« Where the owner is identified as a Body Corporate, a Body Corporate Seal and Body
Corporate resolution consenting the proposal & two signatures (one of which must be the
chairperson OR
