Incident notifications are completed on the Incident Notification Form IMS-05-20. This form is to be used for all
incidents including near misses. All incident forms are to be accompanied with a Rectification Action Plan IMS-
05-69 to address the Corrective and preventive measures identified, as a result of all incidents on this project.

Records relating to incidents are kept in the PCP and emailed to the Compliance department. All incidents are
registered in Shadforth viewpoint system, with all documentation pertaining to notifiable incidents kept for a
minimum of two years.

Incident Investigation

Where an incidents severity is classified as moderate, major, or catastrophic and/or as deemed necessary by
the Compliance Manager then an Incident Investigation Form IMS-05-19 is to be completed by the WHSE
Advisor or the Project Management Team.

Incident management personnel on this project are competent and appropriately trained in Shadforth incident
investigation process and have completed the Incident Investigations Training Paper_IMS-11-37

Following an investigation, a RAP will be raised outlining any corrective or preventative actions identified from
the investigation. Where the investigation identifies a system deficiency or improvement requirement, this will
be added to the continuous improvement register for follow up by the Compliance Manager. Details relating to
factors that led to the incident and subsequent close out of the RAP and review by Senior Management, will be
undertaken in line with section 17 of the manual and section 20 of this Plan.
