The responsibilities for conducting hazard inspections and risk assessments are delegated to managers,
supervisors, WHSE Advisors, and selected staff as identified. It is all staff member’s responsibility to report any
hazards or risks they identify. As a minimum requirement, employees who participate in HIRAC processes must
have undertaken Shadforth HIRAC training paper. Shadforth HIRAC processes are reviewed as part of the
measurement, analysis, and improvement processes set out in this plan

Take 5’s

Take 5’s exist as a supporting tool to Shadforth risk assessment procedure. Take 5’s will be provided to
employees and subcontractors where a risk assessment of the project has been undertaken and a need for
them to be implemented has been identified. They will be implemented where there is a contractual requirement
or client obligation to do so. Take 5’s can be conducted prior to or during an activity where a worker has identified
a hazard or risk associated with the task. Where a hazard has been identified the findings must be discussed
with the person in charge of the work area and a risk assessment must be conducted on the Take 5 Form IMS-
05-51. Once completed the risk assessment must be signed off by an appropriate supervisor and the worker/s
undertaking the task. All Take 5’s must be collected and stored in the PCP. Take 5’s will be addressed in the
same manner as Risk Assessments by the WHSE Advisor.

Safe Work Method Statements

Shadforth acknowledge the statutory requirement to provide safe work method statements (SWMS) for high risk
activities. As such, Shadforth have developed SWMS and supporting procedures for high risk activities
associated with Shadforth tasks. The supporting procedures displayed in the site office and in the PCP.

Generic SWMS are developed following the projects pre-commencement meeting, these are developed by the
WHSE Advisor in consultation with the workers, the Project Management team and stakeholders involved in the
project. Evidence this has occurred is provided in SWMS Register and on the SWMS. Before commencement
of any works the generic SWMS are reviewed and / or where applicable Sub-Contractors SWMS are reviewed
and entered onto the Project Specific SWMS Register IMS-05-75.

It is the responsibility of the Supervisor to update the SWMS acknowledging that a review has been undertaken
with relevant workers. Subsequent reviews must be undertaken, and documented on the Project Specific SWMS
Register, stored in the PCP.

It is the responsibility of the project’s WHSE Advisor to ensure reviews are undertaken, documented, and any
changes passed onto the Compliance Department. Updates to the project risk register are made as a result of
this practice.

The monitoring of SWMS will occur through safety interactions and observations. Verification of the correct
implementation of SWMS will be attained by regular audits of SWMS on any given day.

PPE

Shadforth will provide, monitor and educate workers on the use of PPE through management of SWMS as
referred to in this section of this plan. Monitoring and enforcement of PPE use will be undertaken through regular
site audits and inspections by Management, Supervisors and WHSE Advisors

COVID-19 - Coronavirus

Shadforth will monitor and educate workers on the use of appropriate hygiene measures and social distancing
to reduce the spread of COVID 19 on all our projects. This will be done through information sessions, inductions,
daily pre-starts and toolbox talks. Monitoring and enforcement of appropriate hygiene will be undertaken through
regular site toolbox talks and audits by WHSE Advisors.
